new-client-form.docFrequently Asked Questions
Welcome to Berwick Animal Hospital! We are very excited to welcome you and your pets as our patients. Whether you are a long time client, new to the area or are getting a new addition to your family, we are happy to get you acquainted with our clinic.
How do I establish my pet as a patient at Berwick Animal Hospital?
The best way for us to meet your furry family and establish you as clients is to setup a New Patient Consult with one of our doctors. Call us anytime at 912-232-4249 to setup an appointment, or stop by our front desk in person. Download our New Client Registration Form to ensure we have everything we need for a great first visit.
Do I need to make an appointment?
To ensure we are able to see our scheduled appointments on time, we do prefer, and strongly encourage you make an appointment to be seen with your pet. Walk-ins are accommodated between our scheduled appointments and we cannot guarantee a wait time. We also offer emergency services during our regular hours. Please try to arrive 10-15 minutes early for your appointment to fill out paperwork and ensure we have received previous records on your pet.
I just got a new Puppy/Kitten. When do they need to come in?
As soon as you can schedule a visit after bringing a new pet home! Even if they are not yet due to start or continue vaccines, a wellness exam is recommended and provides us the opportunity to set a vaccination schedule fitting your new pet's age, breed, and future activities. We highly recommend starting a Puppy or Kitten Plan. These packages range based on age and inclusions and provide the best deal to get your new addition started off on the right foot.
I just need Medications or Preventatives refilled for my pet. Do I need to be a client to fill these?
Yes, we do require a patient-doctor relationship in order to fill medications for your pet, which means we need to see your pet at least every year. This ensures we know you, your pets, and what is best for them to be taking in the way of preventatives and medications. No preventative or medication is without its possible side effects, and we want to make the safest and most complete recommendations.
Do you need my pet's records ahead of time?
It is highly recommended and appreciated. Getting vaccine and medical history is very valuable to us so we can be best prepared to make the most complete recommendations for the care of your pet and it will also significantly shorten your wait times during your visit. Records can be e-mailed to us at [email protected]. We are also able to receive records via fax at 912-232-4259 , though this can sometimes be a considerably less reliable form of transmission. Please have your veterinarian email or fax us your pet's complete medical history including vaccines, detailed medical notes, lab results, medications with directions, diagnoses and all other pertinent medical information or let us know their contact information and we will be happy to request your records for you!
How much will my visit cost me?
Please understand that until the veterinarian is able to examine a pet, it is impossible for to give a precise estimate for your pet's care. The following serves as an outline for examination/consultation fess:
Wellness visit exams for patients with no issues and clients with minimal concerns start at $55 and vaccines range from $18-$25 for 1-year vaccines, with the option of some 3-year vaccines based upon eligibility (pricing varies between canine and feline options).
Exams to address concerns, illnesses or injury, as well as some same day appointments, urgent care exams and emergency exams range from $65-$105. Please give our office a call if you have specific questions on pricing.
If the veterinarian has diagnostic or treatment recommendations for your pet at the time of their visit, each client will be presented with a treatment plan outlining all estimated costs and will be asked to authorize those treatments and acknowledge their understanding of our financial policies by signing a consent form. This procedure helps eliminate miscommunications and ensures that our clients can make informed decisions about the care of their pets.
Deposits are required for most hospitalized and surgical patients. Estimates for all anesthetic procedures should be presented at the time of consultation, or in the case of same-day procedures, prior to hospital admission.
